This workflow was designed to deliver routine screening and quantitation while also enabling retrospective data mining to identify unknown or emerging compounds in circulation.</p><p><strong>Here is what makes this approach compelling:</strong></p><ul><li><strong>High sensitivity and resolution:</strong> The Thermo Scientific Q Exactive HF hybrid quadrupole-Orbitrap system enables faster scan speeds and precise mass measurement, which boosts confidence in detecting drugs, even at low levels, in complex postmortem matrices.</li><li><strong>Robust routine productivity:</strong> Running routine screens in approximately 12.5 minutes significantly speeds workflow throughput, which is critical as labs face growing caseloads.</li><li><strong>Retrospective analysis:</strong> One of the biggest advantages of HRAM data is the ability to re-interrogate stored data files for novel psychoactive substances after the initial screen is complete. This capability eliminates the need to re-run samples, saving time and preserving limited sample material.</li></ul><p>A standout example from the technical note was the identification of <strong>bromazolam</strong>, a designer benzodiazepine that was not part of the original routine panel. Through retrospective analysis, its prevalence in postmortem cases was revealed and quantified, demonstrating the real-world value of full-scan HRAM data.</p><h5><strong>Supporting broad drug panels</strong></h5><p>As highlighted in a previous THC tips and tricks blog post, cannabinoids and their metabolites can be challenging when analyzed alongside other drugs of abuse. Sharper, Faster, Smarter: HRAM Orbitrap Mass Spectrometry Meets Postmortem Toxicology

<p><strong>A laboratory will occasionally have a poor performance in a PT scheme. When this occurs, the laboratory should acknowledge it, carry out an investigation and document a review of possible causes, even if it decides not to take any specific action. The purpose of this leaflet is to advise laboratories on how to best address such events. A good management of the poor performance in a PT can save you time and money.&nbsp;</strong></p><h4><strong>Evaluation of poor performance&nbsp;</strong></h4><p>Every unsatisfactory performance score indicates a problem which needs to be investigated. In addition the laboratory should set its own criteria for launching an investigation with regards to other indicators of possible poor performance, such as questionable performance scores or observed trends.</p><h4><strong>Root cause investigation&nbsp;</strong></h4><p>The depth of the investigation will depend on several factors, such as the importance of the analysis, the frequency of unsatisfactory results and evidence of trends.&nbsp;</p><p>The laboratory should check whether the PT report gives an explanation for the unsatisfactory performance. If no reason is provided, a stepwise approach is preferable to maximize the chances of determining the root cause of the problem. This approach is described in <strong>Annex B of the Eurachem Guide [1]</strong>.</p><p><img src="https://storage.googleapis.com/labrulez-bucket-strapi-h3hsga3/Eurachem_How_to_investigate_poor_performance_in_proficiency_testing_Annex_B_be140d5b39.jpg" alt="Eurachem: How to investigate poor performance in proficiency testing - Annex B" srcset="https://storage.googleapis.com/labrulez-bucket-strapi-h3hsga3/thumbnail_Eurachem_How_to_investigate_poor_performance_in_proficiency_testing_Annex_B_be140d5b39.jpg 153w," sizes="100vw" width="726" height="740"></p><p>The investigation should consist of the following steps and involve the personnel that performed the analysis and where approporiate, also involve the laboratory management:&nbsp;</p><ol><li>Analyse the raw data, internal quality control data, any trend from previous PT rounds and the overall performance of the participants in that round;&nbsp;</li><li>After investigation is complete, make a plan for corrective action and consider implications for previous test results;&nbsp;</li><li>Execute and record the corrective action(s);&nbsp;</li><li>Check whether the corrective action(s) was effective.&nbsp;</li></ol><h4><strong>Causes for poor performance&nbsp;</strong></h4><p>The causes of poor performance can be grouped into three categories:&nbsp;</p><ol><li><strong>Clerical errors </strong>are not directly linked to the laboratory’s technical competence, but can underline that the laboratory may have a potential problem when reporting results to its customers. Clerical errors can include transcription errors, mislabelling, incorrect units and decimal point errors. Identifying them is an important first step of an investigation. If such errors are a regular cause of poor performance, the investigation should focus on the related aspects of the quality management system.&nbsp;</li><li><strong>Technical problems</strong> can occur at any step of the analytical procedure. If the investigation does not enable the laboratory to identify the root cause, it may be necessary to review the method validation.&nbsp;</li><li><strong>Problems related to the PT scheme.</strong> Poor performance could also be due to the PT scheme not being entirely appropriate. The Eurachem Guide [1] provides information on selecting an appropriate PT scheme. In other cases, a problem may have occurred with the PT items. The laboratory is encouraged to discuss its findings with the PT provider.&nbsp;</li></ol><p>The “Fishbone diagram” or “<i><strong>5 Whys</strong></i>” are useful tools to investigate the root cause of a problem.</p><h4><strong>Case studies&nbsp;</strong></h4><h5><strong>Example 1</strong></h5><p>A <strong>medical laboratory </strong>has provided results in a PT scheme for tumor markers in serum with a 20 % deviation from the assigned value, although the daily internal controls performed well. The laboratory investigates the process and discovers the mistake was due to an incorrect unit conversion factor being introduced for PT results when the IT system was last updated. Laboratory investigations showed an issue in incubation time and temperature applied by the analyst. He currently serves as the Director of Immuno-Proteomics at Pure MHC, LLC.</p><p>Dr. McMurtrey has studied HLA and its role in immunity since 2005. He is a member of American Society for Histocompatibility and Immunogenetics (ASHI) and holds the distinction of having won four ASHI Top Scholar Awards, more than any other scientist in the history of ASHI. Dr. McMurtrey is most notable for his work on West Nile virus epitope discovery and validation with HLA-A*02:01 and B*07:02; epitopes he discovered have been successfully tested in a WNV vaccine (collaboration with Drs. Hansen and Diamond at Washington University). His most recent accolades come from discovery of MR1 ligands from Mycobacterium tuberculosis (Mtb) as well as over 200 novel ligands from Toxoplasma gondii utilizing Deep Ligand Sequencing (DLS).</p><p>Dr. McMurtrey has expertise in BSL-3 containment techniques, protein purification, HLA ligand isolation, HPLC separation, mass spectrometric analysis, ELISPOT, and multicolor flow cytometry as well as provides bioinformatics expertise. To date, he has directed mass spectrometry (both IDA and SWATH) acquisition as well as comparative analysis and interpretation for a wide variety of MHC ligands including those from tuberculosis, toxoplasmosis, influenza, WNV, HIV and multiple cancers. Dr. Curtis McMurtrey oversees DLS acquisition and analyses and also manages database library collection.</p><p><strong>Presenter: </strong>Jason R. Krawic, Ph.D. (Business Development Manager, Pure MHC, LLC, Oklahoma City, OK, USA)</p><p>Dr. Krawic earned his Master’s degree in Biomolecular Sciences from Central Connecticut State University in 2012 before returning to academia and earning his Ph.D. in Microbiology and Immunology from the University of Oklahoma Health Sciences Center in 2024. He currently serves as the Director of Business Development at Pure MHC, LLC.</p><p>Dr. Krawic began his scientific career teaching Microbiology, Biology, and Chemistry to community college students in Connecticut and Oklahoma. He has studied HLA and non-classical MHC molecules since 2018. He is most recognized for discovering multiple isomeric conformations of MR1 ligands, as well as his work in MHC Class I and Class II ligands and cancer immunotherapies.</p><p>Dr. Krawic’s expertise ranges from classical microbiology and bacteriology to HPLC separation, protein purification, negative ion mode small molecule mass spectrometry, DDA, DIA and MRM acquisition and a variety of bioinformatics tools. In addition to his scientific career, Dr. Krawic has more than a decade of experience in sales and retail management. With this skill set he combines his science and sales backgrounds to serve Pure MHC’s customer base.</p><p><strong>Presenter: </strong>Kris Looney, MBA (President, Pure MHC, LLC, Austin, TX, USA)</p><p>As President of Emergent Technologies, the manager of Pure MHC, Kris Looney is responsible for conceiving and executing key business initiatives, developing strategic plans, and managing business processes including accounting, marketing, intellectual property, and business development. Kris has more than two decades of experience as a business generalist with a background in start-ups, management, accounting, marketing, negotiation, and sales.</p><p>Before joining Emergent, Kris served as the COO and Interim CEO of the American Red Cross of Central Texas, where he balanced multiple stakeholder needs with service responsibilities in a competitive philanthropic environment.
