<p><strong>Michal Holčapek, a researcher in Analytical Chemistry of the Faculty of Chemical Technology of the University of Pardubice has won the Czech Brains Jury’s Extraordinary Award for 2024. With the Czech Brain Award (Česká hlava) being the most prestigious research award in the Czech Republic, Michal Holčapek won the award for his research into lipids and discovery of a method that can diagnose the pancreatic cancer from blood analysis only.</strong></p><p><strong>The 2024 award ceremony was broadcast live by the </strong><a target="_blank" rel="noopener noreferrer" href="https://www.ceskatelevize.cz/porady/10213906625-ceska-hlava/22454416015/"><strong>Czech Television</strong></a><strong>.</strong></p><p>The Czech Brains Award is <strong>the most prestigious award</strong> available to researchers in the Czech Republic. The award has been been presented in several categories since 2002. Since 2007 the Czech Brains Initiative has organized a similar award for secondary-school students called <strong>Little Czech Brain</strong> (Česká hlavička). Last year, <strong>Ing. Barbora Kamenická, Ph.D., a graduate from the University of Pardubice, received the award in the Doctorandus category for technical sciences.</strong></p><p><strong>Professor Michal Holčapek has been researching lipid analysis for a long time with special focus on the detection of some cancer types.</strong> Pancreatic cancer is one of the most aggressive tumours with 80% of the cases being diagnosed too late for the therapy to be effective and thus a 5-year survival rate of less than 10%. For more than 20 years, Professor Holčapek’s team has focused on lipidomic analysis using mass spectroscopy combined with liquid or supercritical fluid chromatography, which enabled them to develop a number of validated analytical methods for detailed description of lipids in biological samples. Thanks to their research, it was possible to develop new screening methods to improve early diagnosis of pancreatic cancer.</p><p>The method is based on lipidomic analysis of blood, which determines the concentrations of more than 150 lipids. Thanks to the data from patients with pancreatic cancer and healthy individuals, a statistical model is created that can determine <strong>whether the patient suffers from the cancer with a certainty of 95%</strong>. The blood test has a number of advantages: it only requires a blood sample and it is reliable to detect even early stages of cancer and thus diagnose the cancer even before the onset of the first symptoms. In addition, similar concentrations of lipids have been observed in other cancer types, showing potential of a universal test for more types of cancer.</p><p><strong>Lipidica, a.s., a spin-off company founded by the University of Pardubice and FONS JK Group, a.s. is conducting the clinical trials</strong> in cooperation with Masaryk Oncological Institute in Brno, Olomouc University Hospital and another 13 hospitals across the Czech Republic. If the method is proved to be accurate and reliable in clinical testing, it can become a basis for national screening programme for those with high risk of pancreatic cancer, which would be unprecedented worldwide. Ventricular arrhythmias are considered as responsible for 75% to 80% of sudden cardiac deaths. Therapy of the disease include ablation, use of cardioverter or defibrillator and pharmacological treatment where the key for successful therapy lays in maintaining the necessary concentration of drugs. The goal can be achieved by therapeutic drug monitoring (TDM). Developed assay named “CardioCarePack” is based on the use of capillary blood collected and dried at home by a patient with volumetric absorptive microsampling (VAMS), quantitative analysis of selected drugs and their metabolites and a telemedical system that integrates all data between a doctor, patient and laboratory supporting the therapy process. CardioCarePack is ready for commercialization.</p><h4><strong>Methods</strong></h4><p>Blood collected with a <a target="_blank" rel="noopener noreferrer" href="https://lcms.labrulez.com/products?distributorItems=315&amp;search=MITRA"><strong>MITRA® microsampling device</strong></a> (20 µl) is prepared with full validated protocol that include sonication with 0.1% formic acid in water followed by acetonitrile extraction, evaporation under nitrogen stream and resuspension in LC mobile phase. Sample is subjected to LC-MS/MS analysis in positive ionization MRM mode with QTRAP 5500+ mass spectrometer (SCIEX) or similar one. Analysis covers simultaneous quantitation of 15 drugs: Sotalol, Metoprolol, Bisoprolol, Propafenone, Carvedilol, Nebivolol, Amiodarone, Digoxin, Perindopril, Ramipril, Eplerenone, Spironolactone, Zofenopril and 2 metabolites: 5-hydroxypropafenone and Desethylamiodarone. Results are placed in a server-based telemedical system where depending on accession rights history, doses, therapeutic index flagging, cardiograms, sample tracking and many other data are available for patient, doctor and laboratory staff, respectively.</p><h4><strong>Preliminary Data</strong></h4><p>The project involved more than 300 patients who had been monitored for 2 years during regular pharmacological therapy. Every half a year during a visit in medical facility venous blood for serum testing and capillary blood on the MITRA® sorbent were collected. Between the visits patients were collecting samples by VAMS themselves at home. All samples were analyzed by fully validated procedure where among the others linearity (r ≥ 0.995), accuracy (80 – 120%), reproducibility (%CV ≤ 15%) for both MITRA® and serum samples were calculated. The LC-MS/MS method developed for 17 compounds covers therapeutic range of the tested drugs: 0.25 – 25 ng/ml (Digoxin and Nebivolol) 2.5 – 250 ng/ml (Metoprolol, Bisoprolol, Propafenone, Carvedilol, Perindopril, Ramipril, Spironolactone, Zofenopril and 25 – 2500 ng/ml (Sotalol, Desethylamiodarone, Eplerenone, Amiodarone, 5-hydroxypropafenone). Results generated during LC-MS/MS analysis were analyzed with SCIEX OS 1.7 software.</p><p>On the basis of samples collected during 4 visits in medical facility a correlation between drugs concentration in venous blood (serum) and capillary blood (MITRA®) (S/M) was calculated. For some compounds S/M ratio was close to 1 (ex. Sotalol – 0.95, %CV – 5.12%, Nebivolol – 1.03, %CV – 3.17%) or moderately to more than 2-fold different (ex. Digoxin – 0.69, %CV – 1.46%, Ramipril – 1.18, %CV – 11.1, Perindopril – 1.86, %CV – 4.73%, Amiodarone – 2.17, %CV – 10.78). The correlation factors are statistically significant (p &lt; 0.05) and can be used for accurate concentration estimation in venous blood.</p><p>Developed telemedicine system integrate CardioCarePack service – a patient, doctor and LC-MS/MS laboratory. The system helps in Doctor’s supervision over the patient’s condition on the basis of collected within the system data ex.: autonomous verification of the therapeutic index with flagging, visualization of results, tracking samples on the patient-doctor line, schedule of visits, results history and many more.</p><h4><strong>Novel aspect</strong></h4><p>A modern telemedicine system for TDM of cardiological drugs based on LC-MS/MS analysis of samples collected at home with VAMS.</p>

Because pharmaceuticals are designed to be effective even at low concentrations, these specific contaminants can exist in the environment at extremely low levels. As with pesticides, these compounds can accumulate in different parts of the ecosystem. Since conventional wastewater treatment plants cannot adequately remove these substances, they are released into the environment, including surface and groundwater, and consequently into drinking water sources.</p><h5><strong>Pharmaceuticals as "New" Pesticides...</strong></h5><p>Pharmaceuticals represent a broad and chemically diverse group of substances that is continuously expanding and is characterised by a wide range of clinical effects and extensive use, often leading to overuse, in both human and veterinary medicine. From an environmental perspective, they are classified as organic micropollutants due to their presence in water at low concentrations (ranging from ng/L to μg/L). Both the public and experts are concerned not only about their negative impacts on natural ecosystems but also about their presence in drinking water and the related effects on human health. Their contribution to the emergence of antimicrobial resistance (AMR) is also significant, as it greatly complicates the treatment of infectious diseases and represents a major challenge for current and future medicine.</p><p>Drugs can enter the environment through various pathways. One of the primary sources of contamination is wastewater from households and health and social care facilities, which contains human urine and faeces, as well as unused or expired pharmaceuticals. Another significant source is wastewater from the pharmaceutical industry. Consequently, wastewater treatment plants (WWTPs) themselves become sources of environmental contamination, as they are unable to effectively remove pharmaceuticals, allowing them to be transported further into the environment [1].</p><p><strong>ALS Laboratories have developed and accredited a multiresidue methods for the determination of more than 100 different pharmaceuticals and their metabolites in various types of water. All of our analytical methods use the LC/MS/MS technique for analytes determination, which provides high sensitivity, selectivity and precision of measurement and allows the determination of target compounds at the very low limits required for residue analysis.</strong></p><h4><strong>2. This technical overview describes an alternative method for implementing an automation method quickly and easily for sample preparation. This includes derivatization of amino acid samples, sample dilution, and preparation of calibration standards using an Agilent 1260 Infinity III Multisampler or Agilent 1260 Infinity III Vialsampler in combination with injector workflows. The injector workflow differs from the conventional injector program in its ability to set up methods faster and easier without error, saving time and cost.</p><h4><strong>4. Waters Corporation:&nbsp;Evaluation of Modern and Legacy HPLC Systems for the Size-Exclusion Analysis of Aggregation and Fragmentation in a Monoclonal Antibody Drug</strong></h4><ul><li>Application</li><li><a target="_blank" rel="noopener noreferrer" href="https://lcms.labrulez.com/labrulez-bucket-strapi-h3hsga3/720008614_en_f03ac5dc38.pdf"><strong>Full PDF for download</strong></a></li></ul><p>Size-exclusion chromatography is a widely used characterization technique for the analysis of aggregation and fragmentation of monoclonal antibody drugs. Traditional high-performance liquid chromatography systems have a large amount of extra-column dispersion which makes size-exclusion data analysis difficult due to band broadening and increased peak tailing. Additionally, biologic analytes may experience non-specific adsorption throughout the chromatographic flow path, further increasing the effects of peak tailing. The Alliance™ iS Bio HPLC System addresses both concerns by offering lower system dispersion than legacy HPLC systems alongside a bio-inert flow path employing MaxPeak™ High-Performance Surfaces Technology.</p><p>In this work an Alliance iS Bio HPLC System, an Alliance e2695, and a competitive Bio-inert HPLC system are compared using a size-exclusion method for analysis of monoclonal antibody aggregation and fragmentation. Dispersion on each system is determined and the Alliance iS Bio HPLC is shown to have the lowest 4σ dispersion. This translates to lower peak tailing, tighter peak width, and a more accurate measurement of low- molecular weight species of infliximab due to lower monomer peak tailing and better monomer resolution.</p><h5><strong>Benefits</strong></h5><ul><li>Biocompatible system for use in salty bio-applications</li><li>MaxPeak HPS for reduced off-target analyte interaction results in less peak tailing</li><li>Low system dispersion results in better fragmentation determination</li></ul><h4><strong>5. Shimadzu: Rapid Screening of 501 Emerging Contaminants in Water Using Polarity-Extended Liquid Chromatography-Mass Spectrometry</strong></h4><ul><li>Application</li><li><a target="_blank" rel="noopener noreferrer" href="https://lcms.labrulez.com/labrulez-bucket-strapi-h3hsga3/an_03_lcmsms_919_en_1790a25e03.pdf"><strong>Full PDF for download</strong></a></li></ul><h5><strong>User Benefits</strong></h5><ul><li>Polarity-Extended Liquid Chromatography combines hydrophilic interaction chromatography and reversed-phase chromatography to achieve simultaneous analysis of polar and weakly polar compounds.</li><li>This method can detect 501 emerging contaminants, offering a broad scope in terms of both quantity and variety.</li><li>The water sample is concentrated using freeze-drying method, effectively recovering both polar and non-polar compounds are recovered well.</li></ul><h5><strong>Introduction</strong></h5><p>In recent years, water pollution has become increasingly serious, and the problem of emerging pollutants in water has attracted much attention. Traditional analytical methods for emerging pollutants mainly focus on 1-2 categories of emerging pollutants and pay less attention to strongly polar pollutants. The Polarity-Extended Liquid Chromatography, through patented technologies such as polarity splitting, online dilution, and dual gradients, combines a Hilic column (first dimension) and a C18 column (second dimension), which can broaden the polarity analysis range of compounds and achieve simultaneous analysis of polar and weakly polar compounds. Fig. 1 shows the Polarity-Extended Liquid Chromatography-Mass Spectrometry System.</p><p>In this paper, a method for rapid screening of 501 emerging pollutants was established by using Shimadzu's Polarity-Extended Liquid Chromatography-Mass Spectrometry System. This method solves the problem of simultaneous analysis of strong and weak polar pollutants.</p><h5><strong>Conclusion</strong></h5><p>In this study, a method for detecting emerging pollutants in water by combining the Polarity-Extended Liquid Chromatography and LCMS - 8050 series coupling system was established. This method combines hydrophilic interaction chromatography and reversed - phase chromatography to achieve the simultaneous separation of polar and non - polar compounds with a single injection. The detection of 501 emerging pollutants was achieved within 28 minutes. The linearity and repeatability of the method meet the quantitative requirements. After pretreatment operations such as freeze - drying, re-dissolution, and nitrogen blowing of the samples, the recovery rates for most compounds were satisfactory. This method has been successfully used for the rapid screening of new pollutants in tap water and sewage. The combination of Polarity-Extended Liquid Chromatography and mass spectrometry enables the one-injection analysis of compounds with different polarities without significantly increasing analysis time, making it suitable for the rapid screening of emerging pollutants in water.</p><h4><strong>6.
